    I hereby grant the Park Board of Trustees of the City of Galveston (Park Board) and Visit Galveston permission to use my likeness in a photograph or video (Photo or Video) in any and all of its publications, websites, and social media outlets without payment or any other consideration.

    I grant to Park Board and Visit Galveston ownership of these Photos and Videos. I understand and agree that these materials will become the property of the Park Board and Visit Galveston and will not be returned. I grant to Park Board and Visit Galveston all rights of every kind to the Photos and Videos, including without limitation, the right to utilize the Photos and Videos anywhere, an unlimited number of times, in perpetuity, and in any and all media, whether now known or hereinafter invented. All rights, including copyright in the Photos and Videos, shall remain vested in Park Board and Visit Galveston.

    I hereby irrevocably and for all times, authorize the Park Board and Visit Galveston to edit, alter, copy, exhibit, publish or distribute these Photos or Videos for purposes of promoting Galveston Island or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the Photos or Video.

    I hereby hold harmless and release and forever discharge the Park Board and the Visit Galveston from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

    This Photo/Video Release form is governed by the laws of the State of Texas.

  4. Liability Release Waiver

    In signing this for myself or the named participant, I understand that participation in the Park Board of Trustees of the City of Galveston cleanup, as in any cleanup event, may involve hazardous activity. I agree to accept and assume all risk and danger incidental to the cleanup occurring prior to, during, or subsequent to the actual cleanup, including but not limited to, the risk of physical injury, mental injury, emotional distress, trauma, death, contact with other participants, equipment failure, inadequate safety equipment, the effect of weather including extreme temperature or conditions, traffic, contact with motor vehicles of all types and descriptions, contact with the trash of unknown origin, or conditions of the shoreline. I waive any and all specific notice of the existence of the risks and hazards. In consideration of being allowed to participate, I, for myself and anyone acting on my behalf, agree to absolve the Park Board of Trustees of the City of Galveston, Visit Galveston, the City of Galveston, board, staff, corporate event sponsors, participants, donor, site coordinators, park districts, beach authorities and their representatives, singly or collectively, of all blame for any injury, misadventure, harm, loss, or inconvenience suffered as a result of taking part in this cleanup or in any of the activities associated with it.

  6. I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS PHOTO/VIDEO AND LIABILITY RELEASE AND ASSUMPTION OF RISK BY READING IT BEFORE I SIGNED IT ON BEHALF OF MYSELF AND MY HEIRS.

    I am 21 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.

  8. Electronic Signature Agreement*

    By checking the "I agree" box below, you agree and acknowledge that 1) your form will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
